Salary:$30,000 - $45,000 per annum
Job Description:
We are seeking a dedicated and compassionate Reablement Care Professional to join our team in the Royal Borough of Windsor and Maidenhead. As a member of our multidisciplinary team, you will play a vital role in supporting individuals to regain their independence and confidence through our reablement program.
Main Responsibilities:
- To deliver person-centered support to residents of RBWM in their own homes.
- To work closely with the therapy team to develop and implement personalized care plans that meet the unique needs of each service user.
- To provide education and training to family members and caregivers on how to support the service user's recovery and independence.
- To maintain accurate records and reports to ensure seamless communication between healthcare professionals and stakeholders.
Person Specification:
- Experience of working with vulnerable adults.
- Full UK Driving Licence.
- Willingness to undertake Care Certificate.
