Job Title: Pediatric Community Nurse - Complex Care Specialist
Job Summary:
We are seeking an experienced Pediatric Community Nurse to join our team at Thornbury Community Services. As a Pediatric Community Nurse, you will be responsible for delivering high-quality care and support to young people with complex needs in a community-based setting.
Key Responsibilities:
- Deliver bespoke care and support to young people with complex needs
- Work collaboratively with the client's family and other healthcare professionals to achieve personal goals and improve quality of life
- Provide education and training to the client and their family on complex care needs and management
- Contribute to the development and implementation of care plans that meet the individual needs of the client
Requirements:
- Minimum of 6 months' experience in the last 2 years as a Pediatric Community Nurse
- Recent experience in key skills such as nasopharyngeal suction, mechanical cough assist, and care and use of gastrostomy
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
