Nurse Case Manager Job Summary:
We are seeking a highly skilled Nurse Case Manager to join our team in Birmingham. This is a temporary NHS contract that offers an exciting opportunity to work in a dynamic and supportive environment.
Key Responsibilities:
- Apply expertise in the National Framework for NHS Continuing Healthcare and Nursing Funded Care.
- Conduct holistic assessments of patients with complex healthcare needs.
- Effectively communicate complex information to patients and carers.
- Maintain excellent interpersonal skills to foster collaborative working relationships across NHS and independent sectors.
- Experience with holistic assessments, complex case management, and CHC procedures.
- Proficient in IT (Word, Excel, PowerPoint), able to work independently.
Salary and Benefits: Our salary package includes £37,000-£43,000 per annum, depending on experience, plus a range of benefits.
