Community Health Nurse

We are seeking a highly motivated and enthusiastic nurse to join our Adult Community Health Services team in Greenwich and Bexley Boroughs. As a Community Staff Nurse, you will work autonomously and as part of a team to deliver high-quality nursing care to patients in their home environment.
Key Responsibilities- Deliver high-quality care in the community, working with patients towards rehabilitation, self-management, and independence.
- Conduct comprehensive assessments, develop individualized care plans, and evaluate care to ensure high standards are consistently met.
- Work collaboratively with the wider multidisciplinary team to provide holistic care, addressing physical, emotional, and social needs.
- Develop and maintain strong relationships with patients, families, and carers to ensure person-centered care.
- Participate in regular clinical supervision and reflective practice to maintain professional development and contribute to the growth of junior staff.
