About the Role

We are seeking an experienced Support Worker to join our Home First Team in Kent, working in partnership with Kent County Council and the NHS. As a Support Professional, you will play a vital role in supporting patients to return home following a hospital admission and facilitating early supported discharge from hospital.

Key Responsibilities
  • Provide personal care and rehabilitation to support patients to return home
  • Facilitate early supported discharge from hospital through access to health and care needs assessment at home
  • Work collaboratively with the Home First Team to deliver high-quality patient care and services
Requirements
  • Full UK driving licence (with reasonable adjustments for disability)
  • Sound communication and interpersonal skills (verbal and written)
  • Ability to organise and prioritise own work, moderate IT skills, and ability to manage and prioritise care delivery in a professional manner
