Care Coordinator for Elderly Patients

The successful candidate will work as part of a multidisciplinary team to deliver care and organize structured activities for stroke patients. This will involve planning, implementing, and evaluating social and leisure activities, as well as participating in therapeutic leisure activities on a group and one-to-one basis.

The post holder will also be responsible for assisting patients with personal care, including toileting and meal times. Effective communication and teamwork skills are essential for this role, as well as the ability to work in a busy environment.

Essential requirements include effective written and verbal communication skills, literacy and numeracy skills, and the ability to carry out assigned tasks effectively. The successful candidate will also be required to complete induction training, including the HCSW workbook and mandatory LearnPro training.
