Job Summary

We are seeking a highly skilled and compassionate Specialist Palliative Care Nurse to join our team at St Margaret of Scotland Hospice. As a key member of the Community and In-Patient based Palliative Care Team, you will provide specialist care to patients living at home with life-threatening illnesses and their carers.

Key Responsibilities
  1. Work collaboratively with the multi-professional team to deliver specialist palliative care within the hospice and community settings.
  2. Participate in daily ward rounds, weekly grand ward rounds, and daily patient handovers.
  3. Establish yourself as a resource, facilitator, and role model for specialist palliative care within the hospice and community settings.
  4. Adequately assess and address the needs of patients and their families through direct and indirect care and multidisciplinary working.
  5. Participate in the hospice virtual clinic and deputise for the community palliative care lead as required.
  6. Liaise with the hospice social worker and therapy team to optimise the psychosocial and functional aspects of patient care.
  7. Support families and carers in bereavement, referring appropriately to child and adult bereavement services.
Requirements

The ideal candidate will have a strong background in palliative care, excellent communication and interpersonal skills, and the ability to work effectively within a multi-professional team.
