About the Role
We are seeking a highly skilled and compassionate Health and Wellbeing Coach to join our team at Mayflower Medical Group. As a Health and Wellbeing Coach, you will play a vital role in supporting our patients to achieve their health and wellbeing goals.
Key Responsibilities
• Provide individual support and guidance to patients to improve their mental health and wellbeing
• Work closely with patients to identify their health and wellbeing needs and develop personalized plans to address them
• Collaborate with healthcare professionals and community organizations to ensure seamless care and support
• Utilize social prescribing to develop individual recovery and support planning
• Promote effective and efficient working practices across the team to achieve measurable outcomes
Requirements
• Experience working in a healthcare setting, preferably in a mental health or wellbeing role
• Strong communication and interpersonal skills, with the ability to build rapport with patients and colleagues
• Ability to work independently and as part of a team, with a flexible and adaptable approach
• Commitment to ongoing professional development and staying up-to-date with best practices in mental health and wellbeing
