Physician - Geriatric Care Specialist

We are seeking a highly skilled Physician to join our team at JM Chandler Dream Builders. As a Physician - Geriatric Care Specialist, you will be responsible for providing comprehensive geriatric primary care and post-acute care services to our senior living residents.
Key Responsibilities- Conduct thorough medical evaluations and develop treatment plans for patients
- Order and interpret diagnostic tests, prescribe medications, and provide non-pharmacologic treatment modalities
- Collaborate with Advance Practice Providers to manage acute and long-term medical needs of patients
- Maintain accurate and up-to-date patient records and charts
- Provide education and guidance to patients and their families on medications and treatment instructions
- Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) degree
- Current unrestricted license to practice medicine in the state
- Board Certification in Internal Medicine or Family Medicine
- Current DPS and DEA certificates
- Previous experience in geriatrics, primary care, and post-acute care, preferably in senior living communities
- Experience with value-based care and electronic health records (EHRs)
- Strong organizational and interpersonal skills
- Ability to work independently and as part of a team
