Job Summary

We are seeking a highly skilled Physician to join our team at JM Chandler Dream Builders. As a Physician - Geriatric Care Specialist, you will be responsible for providing comprehensive geriatric primary care and post-acute care services to our senior living residents.

Key Responsibilities
  • Conduct thorough medical evaluations and develop treatment plans for patients
  • Order and interpret diagnostic tests, prescribe medications, and provide non-pharmacologic treatment modalities
  • Collaborate with Advance Practice Providers to manage acute and long-term medical needs of patients
  • Maintain accurate and up-to-date patient records and charts
  • Provide education and guidance to patients and their families on medications and treatment instructions
Requirements
  • Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) degree
  • Current unrestricted license to practice medicine in the state
  • Board Certification in Internal Medicine or Family Medicine
  • Current DPS and DEA certificates
  • Previous experience in geriatrics, primary care, and post-acute care, preferably in senior living communities
Preferred Qualifications
  • Experience with value-based care and electronic health records (EHRs)
  • Strong organizational and interpersonal skills
  • Ability to work independently and as part of a team
