Senior Community Children's Nurse

Homerton Healthcare NHS Foundation Trust is seeking a highly skilled and dedicated Paediatric Community Nurse Specialist to join our Community Children's Nursing team. As a key member of our team, you will provide specialist nursing care to children in the community, working closely with parents and carers to ensure the best possible outcomes for our patients.
Main Duties- Provide high-quality nursing care to children with varying health needs, including those with life-limiting or life-threatening conditions.
- Work collaboratively with the Community Children's Nursing Team to develop and implement individualized care plans, ensuring seamless transitions between hospital and community settings.
- Facilitate the development of the extended role of the Community Children's Nursing Team, providing teaching, support, and resources to colleagues.
- Communicate complex and sensitive information to parents and carers, ensuring they are confident and competent in caring for their child at home.
- Manage client caseloads, work autonomously, and delegate tasks to junior staff members as needed.
- Contribute to the smooth running of the team, attending discharge planning meetings and providing input on patient care.

Person Specification- Registered Child Teaching/mentoring 998 or equivalent.
- Substantive experience of working with children in the community.
- Ability to communicate complex and sensitive information, manage client caseloads, and work autonomously.
