Job Description:
- We are seeking a highly motivated and empathetic Community Care Assistant to join our team, supporting individuals in their own homes or residential settings.
- The successful candidate will work closely with colleagues and other healthcare professionals to develop and implement tailored care plans that meet the unique needs of each client.
- Key responsibilities include assisting with daily living tasks, maintaining accurate records, and reporting changes or concerns to the management team.
