About the Role

We are seeking a skilled and experienced Band 6 Mental Health Practitioner to join our Multiple Admissions Patient Pathway Team in Hastings. This team works with individuals who present with complex mental health needs, are high users of services, and are at risk of multiple admissions to hospital due to frequent cycles of crisis and difficulties engaging in community support.

Key Responsibilities
  • Conduct clinical assessments, including risk assessments, to inform care planning and safety planning.
  • Work closely with Lead practitioners and third sector services to support therapeutic engagement and recovery.
  • Facilitate network meetings to promote collaboration and best practice.
Person Specification

We are looking for a practitioner with significant post-qualifying experience in mental health, a strong understanding of clinical governance, and experience of supervising staff. The ideal candidate will have a proven track record of working with complex mental health needs, conducting clinical assessments, and providing a range of clinical interventions. They will also have experience of partnership working within and across various statutory and non-statutory teams and agencies.

What We Offer

  • A competitive salary and a £4,000 incentive for Band 5-7 practitioners.
  • A relocation package up to a maximum of £8,000 (subject to terms and conditions).
