Job Title: Care and Support Specialist

Salary: £22,000 per annum (dependent on experience).

Job Summary: We are seeking a dedicated and caring Care and Support Specialist to join our team. As a key member of our team, you will be responsible for providing person-centred care to our service users in their own homes, promoting independence and dignity while maintaining their safety and well-being.

Key Responsibilities:

• Develop and implement individual care plans that meet the unique needs of each service user;
• Provide a range of domiciliary care services, including personal and continence care, managing medication, and household tasks;
• Maintain accurate records and report any changes or concerns;
• Work collaboratively with other healthcare professionals to ensure seamless care delivery;
• Participate in regular team meetings and contribute to continuous improvement initiatives;


Requirements:

• Minimum N/SVQ2 in Adult Health and Social Care (or equivalent professional qualification);
• Literacy and numeracy skills to maintain accurate records;
• Ability to use smartphone apps and computer applications;
• Proven track record in a similar role;
