Job Summary

East:MK Primary Care Network is seeking a clinical pharmacist to join its practice team on a full-time basis. The successful candidate will work collaboratively with the multidisciplinary team (MDT) within the PCN, ensuring safe, evidence-based, and cost-effective prescribing. Key responsibilities will include supporting general practice staff with prescription and medication queries, conducting medicines reconciliation on transfer of care, and working proactively with other healthcare professionals to manage chronic diseases.

Main Duties

  • Work as part of a multidisciplinary team in a patient-facing role.
  • Contribute to chronic disease management, working with other healthcare professionals to improve patient outcomes.
  • Support the delivery of the Network Contract Directed Enhanced Service (DES).
  • Conduct structured medication reviews to proactively manage patients with complex polypharmacy.
  • Provide clinical medicines expertise to patients and practice teams.
  • Improve prescribing practices across the PCN.
  • Contribute to quality and service improvement through clinical audits.
  • Interface with community and hospital colleagues to resolve issues.
  • Support enhanced health-in-care-homes initiatives.

Person Specification

  • Vocational 4-year master's degree in pharmacy.
  • One year of pre-registration training.
  • Registration with the General Pharmaceutical Council.
