About the Role

We are seeking an experienced and skilled Pharmacist to join our team as a Medication Specialist in Primary Care. This is an exciting opportunity to work closely with general practice staff to promote and embed the role of a GP practice pharmacist, and raise awareness of services available to patients.

Main Responsibilities
  1. Participate in the multidisciplinary team delivering high quality pharmaceutical care to patients
  2. Provide interface and discharge liaison support to wards and units within the Care Group
  3. Lead on the provision of clinical pharmacy services to sub-specialties within the Care Group
  4. Audit medicines usage in the Care Group to promote cost-effective prescribing
  5. Work with general practice staff to promote medicines optimisation and high quality prescribing
  6. Process medication eConsults, reconcile medications, and process medication workflow across Hurley Group primary care sites
  7. Provide operational and clinical support to medicines optimisation developments and prescribing initiatives
  8. Collate and feedback on electronic discharge information to improve hospital computerised discharge letter and communication processes

The estimated salary for this role is £60,000 - £80,000 per annum, depending on experience and qualifications.
