Pharmacist Medication Specialist

We are seeking an experienced and skilled Pharmacist to join our team as a Medication Specialist in Primary Care. This is an exciting opportunity to work closely with general practice staff to promote and embed the role of a GP practice pharmacist, and raise awareness of services available to patients.
Main Responsibilities- Participate in the multidisciplinary team delivering high quality pharmaceutical care to patients
- Provide interface and discharge liaison support to wards and units within the Care Group
- Lead on the provision of clinical pharmacy services to sub-specialties within the Care Group
- Audit medicines usage in the Care Group to promote cost-effective prescribing
- Work with general practice staff to promote medicines optimisation and high quality prescribing
- Process medication eConsults, reconcile medications, and process medication workflow across Hurley Group primary care sites
- Provide operational and clinical support to medicines optimisation developments and prescribing initiatives
- Collate and feedback on electronic discharge information to improve hospital computerised discharge letter and communication processes

The estimated salary for this role is £60,000 - £80,000 per annum, depending on experience and qualifications.
