About the Role:
">This is an exciting opportunity to join our dynamic Adult Community Nursing Team as a highly motivated and compassionate Registered Nurse. As a key member of our integrated neighbourhood team, you will play a pivotal role in delivering high-quality care to patients across a seven-day schedule.

- Work collaboratively with co-located health and social care professionals to provide holistic patient care; ">
- Conduct home visits to assess patient needs and develop tailored care plans; ">
- Provide education and support to patients and their families to promote independence and empower them to manage their health effectively; ">
- Liaise with GPs, hospitals, and other healthcare providers to ensure seamless care coordination; ">
- Participate in cutting-edge initiatives designed to prevent hospital admissions and facilitate smooth discharges; ">
- Collaborate with our team leader to identify opportunities for process improvements and contribute to the development of best practices; ">
What We Offer:
">A competitive salary of £32,000 - £43,041 per annum (dependent on experience), plus a range of benefits including annual leave allowance, flexible working opportunities, and protected hours for health and wellbeing activities.
">Requirements:
">To be successful in this role, you must have current NMC registration as a Registered Nurse Adult Branch, effective communication skills, and IT proficiency. A background in community nursing is desirable, but not essential.
