Hub Support Worker

About the Role

We are seeking a skilled and dedicated Hub Support Worker to join our team at Engage Leeds. As a key member of our small, friendly team, you will play a vital role in processing and triaging referrals, offering initial advice, and carrying out short-term prevention work.

Main Responsibilities

  • Support the delivery of the Engage Leeds city-wide housing support service.
  • Process and follow up referrals delegated by the Senior Hub Co-ordinator and Hub Team Leader.
  • Deliver triage interventions and signposting for people being referred and for referring agencies.
  • Undertake needs and risk assessments to ensure safe practice in referral, triage, and community-based interventions.
  • Carry out short-term advice/prevention work around housing support issues.
  • Liaise with colleagues across Engage Leeds to ensure the provision of high-quality and timely support and advice.
  • Update and maintain accurate records within the Gateway case management system.
