About the Role

Aneurin Bevan University Health Board is seeking a dedicated and experienced Registered Nurse to join our innovative nursing team. As a Registered Nurse - Community Care Specialist, you will play a vital role in delivering high-quality care to individuals with complex health needs in their own homes within the community.

Main Responsibilities

  • Deliver care within packages of care at home and work in a role delivering clinical lead presence across the community.
  • Manage individuals with Learning Disabilities and Complex Health needs or have the willingness to learn and facilitate individuals to participate in all aspects of their life.
  • Develop a wide range of clinical skills and work with a superb team of nurses and healthcare support workers.
  • Travel between sites in a timely manner and have the ability to drive patient vehicles.
