About the Role

Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust is seeking an experienced Band 6 Community Nurse Practitioner to join our Newcastle and Gateshead Learning Disability Teams. The successful candidate will be responsible for working with individuals with complex needs, who are at risk of hospital admission, placement breakdown, and presenting as acutely unwell.

The role involves assessing and managing a caseload of patients, providing high-quality care, and collaborating with patients to develop care plans, safety plans, and recovery and wellbeing plans. The successful post holder will be expected to demonstrate a strong commitment to high-quality patient care, good organisational skills, and the ability to prioritise workload.
