Key Responsibilities:

  • Champion promoting peoples' skills and abilities to optimise their independence and maintain their own support networks.
  • Assist in the delivery of the Hoots Service with constant competing demands, undertaking staff supervision and development, contributing to the management of the Teams operational functions and strategic directions.
  • Provide practical care and support to individuals in their own homes, completing risk assessments and effective care planning to meet individual need.
  • Proactively support and encourage joint working across Adult Social Care, primarily between Social Care, Housing, Occupational Therapy, and Health services to ensure services are delivered in accordance with Care Quality Commission requirements and support National Strategic Frameworks and local objectives for performance.

Requirements:

  • GCSE grade C or above in maths and English or equivalent is essential.
