**Job Description:**

We are seeking a caring and dedicated Community Care Worker to join our team in Dundee. As a Community Care Worker, you will provide high-quality support to individuals in their own homes, helping them to maintain their independence and quality of life.

About the Role:

  • Assist and enable individuals with activities of daily living, including bathing, dressing, meal preparation, medication management, and mobility support.
  • Work closely with people, their families, and other professionals to create care plans tailored to their needs and goals.
  • Collaborate with healthcare professionals, social workers, and other team members to ensure a comprehensive and coordinated approach to care.
  • Maintain accurate and thorough documentation of interactions, care plans, and progress.
**Required Skills and Qualifications:**

To be successful as a Community Care Worker, you will need:

  • Previous experience in a similar care setting, preferably within a healthcare or social care environment.
  • A compassionate and empathetic nature, with a strong desire to make a positive impact on individuals' lives.
  • Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
  • Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
  • Ability to work independently, demonstrating initiative and reliability.
