Job Summary

We are seeking an experienced and dedicated Nurse Practitioner to join our team at Northumbria Healthcare - NHCT Northumbria Healthcare NHS Foundation Trust. As a Senior Community Health Specialist, you will play a crucial role in delivering high-quality patient care in the community.

About the Role

The successful candidate will be responsible for assessing, planning, and evaluating client care, including families and carers where appropriate. You will work as an autonomous practitioner, providing expert clinical care for patients within community and rehabilitation settings.

Key Responsibilities

  • To assess, plan, and evaluate clients' care, including families and carers where appropriate.
  • To work as an autonomous practitioner and provide expert clinical care for patients within community and rehabilitation settings.
  • To use clinical skills and non-medical prescribing to support patients to remain at home and avoid admission to hospital.
  • To undertake comprehensive assessment of the physical and psycho-social care needs of frail patients who may also have complex chronic disease, formulating a clinical management plan.

Requirements

To be successful in this role, you will need:

  • A degree in Nursing or equivalent qualification.
  • Registration with the NMC as a Registered Nurse.
  • Postgraduate education in a relevant field (e.g., district nursing, community health).
  • Demonstrated experience in acute assessment, long-term conditions, frailty, physical assessment, clinical reasoning, diagnostic reasoning, and independent prescribing.
  • Excellent communication and interpersonal skills.
