Job Description

The Clinical Support Worker Higher Level (UCR Home First) role is a challenging and rewarding opportunity to support the urgent community response multidisciplinary team working across the South Tees locality. This position has been developed to provide early discharge from hospital when a package of care cannot be sourced, ensuring seamless transitions for patients.

The main duties of this job include supporting with personal care and completion of activities of daily living, as well as therapy rehabilitation to improve patient outcomes. The successful applicant must have a Care certificate or be willing to work towards one if they already hold an NVQ level 3. Additionally, they should possess a Level 3 vocational qualification or short courses and completion of competency workbook equivalent to Level 3 in a range of patient and clinical care duties.
