We are looking for an experienced nurse to provide continued level of care to our patients in a dynamic and diverse range of settings.



About the Role

Work closely as part of the Multi-Disciplinary Team for the care of patients requiring medical and rehabilitation support, predominantly post an acute hospital stay.



Responsibilities


  • Assess care needs and develop patient-centered care plans.
  • Lead in our Nurse-led unit, especially during out-of-hours shifts.
  • Shift work and shift management, working with junior members of staff in all aspects of patient care, participating in training and support.
