Main Responsibilities
- Conduct assessments, care, and treatment for individuals with learning disabilities, mental health issues, and challenging behaviors.
- Work closely with the Intensive Support Team to develop and implement personalized care plans.
- Provide advice and consultation to colleagues on supporting autistic adults.
- Facilitate access to health services through collaboration with primary and secondary health, and other relevant services.
- Develop working relationships with clients, provider organizations, GPs, and local community resources.
Requirements
- NVQ Level 3 qualification or equivalent.
- Experience of working with individuals with learning disabilities, mental health issues, and challenging behaviors.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
