Main Responsibilities

  • Facilitate access to health services through collaboration with primary and secondary health, and other relevant services.
  • Develop working relationships with clients, provider organizations, GPs, and local community resources.

Requirements

  • NVQ Level 3 qualification or equivalent.
  • Experience of working with individuals with learning disabilities, mental health issues, and challenging behaviors.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
