About the Role

We are seeking a dedicated and compassionate Care Manager to join our team at Barchester Healthcare. As a key member of our care home, you will be responsible for providing exceptional care and support to our residents.

Job Description:

In this rewarding role, you will have the opportunity to make a real difference in the lives of our residents. Your primary focus will be on ensuring that every individual receives high-quality care tailored to their unique needs and preferences.

You will work closely with our nursing team to develop and implement personalized care plans, taking into account each resident's medical history, lifestyle, and personal goals. Your attention to detail and ability to communicate effectively will be essential in building strong relationships with our residents and their families.

Requirements:

To succeed in this role, you must be a registered nurse with current NMC registration. Experience in producing well-developed care plans and detailed risk assessments is crucial, as is an up-to-date knowledge of recent clinical practices and regulatory frameworks.

You should be a natural leader who has acted as a clinical mentor, supporting newly qualified nurses through their preceptorship. A strong track record of delivering improvements to achieve quality care is also essential.
