Job Description: Community Care Coordinator - Older Persons

We are seeking an experienced and compassionate individual to join our team as a Community Care Coordinator for Older Persons. In this role, you will work closely with older adults, their families, and healthcare professionals to provide person-centered care and support.

Salary Information: £35,472 - £60,690 per annum (dependent on experience)
