Rough Sleeper Navigator Role

About the Role

We are seeking a skilled and compassionate individual to join our team as a Rough Sleeper Navigator in Exeter. As a key member of our Supported Housing team, you will play a vital role in providing tailored interventions to individuals experiencing homelessness.

Key Responsibilities

  • Develop and implement effective support plans to address the complex needs of rough sleepers
  • Build strong relationships with service users, stakeholders, and community partners to ensure seamless support
  • Collaborate with internal teams to provide comprehensive support services, including housing, health, and employment
  • Conduct regular outreach and engagement activities to identify and support individuals at risk of homelessness
