Job Overview

An exciting opportunity has arisen for a dedicated and compassionate individual to join our Stroke Specific Early Supported Discharge Team in the East BCUHB area.

Key Responsibilities
  • Gathering baseline information to assist therapy assessments.
  • Carrying out therapeutic interventions following initial assessments carried out by physiotherapists and occupational therapists.
  • Progressing transfers and mobility, delivering exercise programmes, and practicing activities of daily living such as washing, dressing, and meal preparation.
  • Working independently to promote wellbeing, safety, and independence within essential activities of daily living.
About the Role

This role is ideal for a motivated and patient-centred individual who is passionate about supporting patients in the community who have recently suffered a stroke.

As a member of our multidisciplinary team, you will work closely with physiotherapists, occupational therapists, and other healthcare professionals to provide holistic assessment and therapy intervention to patients living across Wrexham and Flintshire.

You will have the opportunity to learn and develop with support from experienced colleagues and contribute to the delivery of high-quality patient care.

Requirements

The ability to speak Welsh is desirable for this post, and Welsh and/or English speakers are equally welcome to apply.
