About the Role

We are seeking a highly skilled and compassionate Care Team Leader to join our team at City and County Healthcare Group Ltd. As a Care Team Leader, you will play a vital role in ensuring the delivery of high-quality care services to our clients.

Key Responsibilities
  • Lead a Team of Care Assistants: Supervise and support a team of care assistants to ensure they are providing the best possible care to our clients.
  • Develop Care Plans: Work with clients and their families to develop personalized care plans that meet their unique needs and goals.
  • Monitor and Evaluate Care Services: Regularly monitor and evaluate the quality of care services provided to ensure they are meeting the needs of our clients.
  • Collaborate with Healthcare Professionals: Work closely with healthcare professionals, including doctors, nurses, and therapists, to ensure seamless care coordination.
  • Manage Client Relationships: Build and maintain strong relationships with clients and their families to ensure their needs are met and they feel supported.
Requirements
  • Experience in Care Services: A minimum of 2 years' experience in care services, preferably in a leadership or supervisory role.
  • Qualifications and Training: A relevant qualification in care services, such as a diploma or degree in social care or a related field. Additionally, you should have completed any necessary training, such as first aid or manual handling.
  • Communication and Interpersonal Skills: Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and healthcare professionals.
  • Leadership and Management Skills: Strong leadership and management skills, with the ability to motivate and support a team of care assistants.
