About the Role

We are seeking a dedicated and compassionate Reablement Support Worker to join our team at Stockton-on-Tees Borough Council. As a Reablement Support Worker, you will play a vital role in supporting individuals to regain their independence and confidence in their daily lives.

Key Responsibilities
  • Support individuals in their own homes on a short-term basis to regain independence and confidence
  • Assist with personal care and daily living tasks, promoting independence and self-reliance
  • Work collaboratively with clients, families, and other healthcare professionals to develop personalized care plans
  • Provide emotional support and empathy to clients, promoting a positive and empowering experience

Requirements
  • A full driving license and access to a vehicle for home visits
  • Experience of working in a care setting and/or NVQ Level 2 in Adult Social Care or equivalent
  • Good verbal and written communication skills
