About the Role

We are seeking a highly skilled and experienced Community Healthcare Specialist to join our team at Buckinghamshire Healthcare NHS Trust. As a key member of our Rapid Response & Intermediate Care service, you will play a vital role in providing expert interventions and rehabilitation to patients in their own homes.

Key Responsibilities
  • Provide comprehensive specialist assessment, review, and case management for patients and carers.
  • Develop and implement personalized care plans to promote health gain and maximize independence.
  • Work collaboratively with a range of professionals to ensure timely and effective care delivery.
  • Lead and manage the Rapid Response & Intermediate Care team, allocating workloads and supporting staff development.
  • Contribute to the development of clinical pathways and quality improvement initiatives.
Requirements
  • Registered Allied Healthcare Professional with HCPC registration.
  • Degree or Diploma in Physiotherapy or Occupational Therapy.
  • Experience of recent Continuous Professional Development.
  • Relevant postgraduate clinical training in areas such as long-term conditions, respiratory disease, and end-of-life care.
Desirable Qualifications and Experience
  • Experience of working in a community or in-patient setting for a minimum of 2 years post-registration.
  • Experience of working within multi-agency/multi-disciplinary teams.
  • Electronic clinical systems experience.
  • Day-to-day staff management and case allocation experience.
  • Experience of working with single assessment processes.
Skills and Abilities
  • Profession-specific clinical assessment, planning, delivery, and evaluation skills.
  • Excellent written and verbal communication skills.
  • Presentation skills.
  • Self-awareness and ability to motivate staff.
Special Circumstances
  • Valid and current driving license and car to travel to locations throughout Buckinghamshire.
