Job Title: Community Outreach Coordinator

Key Responsibilities:
  • Work closely with the Experience of Care team to develop and implement strategies to reduce health inequalities.
  • Collaborate with internal teams to improve patient experience.
  • Signpost and refer to community support services.
Requirements:
  • Substantial experience of working with Gypsy, Roma, and Traveller communities in various settings.
  • Knowledge and experience of current legislation and guidance around Gypsy, Roma, and Traveller communities in health and social care.

The estimated salary for this role is £38,000 - £50,000 per annum, depending on experience.
