Job Overview

The role of a Physiotherapist in our Community Rehabilitation team is to provide high-quality patient care and support in a community setting. As a key member of our team, you will work closely with patients, families, and healthcare professionals to develop and implement effective rehabilitation plans.

Main Responsibilities
  • Conduct thorough assessments of patients' physical and functional abilities to identify rehabilitation needs and goals.
  • Develop and implement individualized rehabilitation plans, incorporating evidence-based practices and patient-centered care.
  • Provide education and support to patients and their families on self-management techniques, exercise programs, and lifestyle modifications.
  • Collaborate with multidisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive care and coordination.
  • Monitor patient progress, adjust rehabilitation plans as needed, and communicate effectively with patients and healthcare teams.
Person Specification

We are seeking a highly motivated and experienced Physiotherapist who possesses excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced community setting. The ideal candidate will have a strong understanding of rehabilitation principles and practices, with the ability to adapt to changing patient needs and priorities.

Requirements
  • Registration with the Health and Care Professions Council (HCPC) as a Physiotherapist.
  • Postgraduate qualification in Physiotherapy or equivalent.
  • Minimum 2 years' experience in a community rehabilitation setting.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
