Job Description:

This is a challenging role for a dedicated Nurse Advisor who has experience in the field of Disease Management in primary care.

About Us:

Salary:

Key Responsibilities:
  • To work in partnership with NHS professionals and patients to deliver high-quality care, aligned with national guidelines, in the management of Type 2 Diabetes.
  • To develop and implement best practices in diabetes care, focusing on education, support, and advice to individuals with Type 2 Diabetes.
  • To have a deep understanding of oral hypoglycaemics, including their optimisation, and be skilled in using primary care systems.
Requirements:

The ideal candidate will be an experienced NMC-registered Nurse with at least 3 years of post-registration experience in managing Diabetes within primary care. They should possess excellent planning and organisational skills, as well as a full UK driving licence.
