Complex Case Manager

We are seeking a highly skilled and compassionate Complex Case Manager to join our team at Routes Healthcare. As a key member of our care team, you will be responsible for managing complex care packages and providing exceptional support to our clients with spinal injuries, acquired brain injuries, motor neuron disease, and multiple sclerosis.
Key Responsibilities- Take ownership of a caseload of complex clients and manage their care package holistically
- Coordinate and lead the transition, start-up, and continuity of all complex care packages
- Develop comprehensive care plans and risk assessments
- Provide support for care packages through observations and mentoring sessions
- Uphold high standards in care and prioritize a person-centred approach
