We are seeking a highly motivated and compassionate individual to join our team as a Community Connect Coordinator. This exciting role will enable you to make a real difference in the lives of stroke survivors and their families in Carmarthenshire.

About the Role

The Community Connect Coordinator will provide personalized support and guidance to new stroke survivors and their carers, empowering them to live independent lives in the community. This will involve:

  • Providing tailored information, advice, and support to meet individual needs.
  • Facilitating connections between stroke survivors and their community, promoting stroke-friendly environments.
  • Collaborating with healthcare professionals, social care services, and community organizations to ensure seamless support.
  • Managing a team of volunteers to amplify the impact of the Community Connect service.

Requirements

To succeed in this role, you must be able to demonstrate your ability to travel across the locality, connect people to their community, and promote stroke-friendly environments. You must also be resident in the UK and have the right to work in the UK.
