Job Summary:
DigiKape is seeking a highly skilled and experienced Social Worker to join our Adults Community/Locality team in Knowsley. As a key member of our team, you will be responsible for providing high-quality social care services to adults in the area.
Key Responsibilities:
- Conduct comprehensive social care needs assessments to identify individual needs and develop personalized support plans.
- Collaborate with service users, families, and other stakeholders to ensure effective support and risk management.
- Develop and implement risk assessments and management plans, including attending MARAMS and other relevant meetings.
- Ensure compliance with safeguarding policies and procedures, including MCA/BI, COP DOLS, and CHC - DST.
- Work closely with other teams and agencies to ensure seamless service delivery and effective communication.
Requirements:
- Proven experience as a Social Worker in a similar role.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong analytical and problem-solving skills.
- Valid driving license and access to a vehicle.
Working Arrangements:
The successful candidate will be required to work 36 hours per week, with a hybrid working pattern that includes mandatory office days on Wednesday and Friday, and other training and duty days as required.
