Job Overview

This is an exciting opportunity to work in our community and primary care occupational therapy service in North Wales. The post will involve collaborating closely with general practitioners and district nursing colleagues in primary care to further develop multidisciplinary team working.

The successful candidate will have experience and a personal interest in complex moving and handling, seating, and posture, and will manage a varied caseload of patients with complex needs.

Main duties of the job:

  • To provide holistic occupational therapy assessments, goal setting, individualized treatment and rehabilitation programs, care planning and delivery.
  • Regular review and discharge of patients.
  • To manage a diverse caseload, seeking support from senior staff when necessary to ensure the provision of a timely, clinically effective service based on evidence-based practice.
  • To be actively involved in, and lead on, projects towards the development and delivery of the service, and contribute to new developments and initiatives.

The ability to speak Welsh is desirable for this post; both Welsh and English speakers are equally welcome to apply.
