Adult Services Assistant

We are seeking a skilled Social Care Practitioner to join our Hospital Teams in Warwickshire. As a member of our team, you will work with adults aged 18 years and above who require social care support, including those with physical disabilities and mental health issues.
Job DescriptionIn this role, you will undertake assessments, create personalized support plans, and review customer progress. You will also work collaboratively with colleagues and other professionals to ensure comprehensive care. The ideal candidate will possess excellent communication skills, empathy, and a commitment to promoting adult wellbeing.
