Community Care Worker

This is a permanent contract role with flexible working hours, supporting individuals in their own homes and enabling them to stay at home safely.
The position is available in South Norfolk locations with both full-time and part-time options considered.
Main Responsibilities- To contribute to the delivery of care pathways for patients with long-term conditions, palliative care, and rehabilitation needs.
- To work within the Urgent Community Response Team to prevent unnecessary admission to hospital.
The post holder will need:
- Work generically applying competency-based treatment techniques/care to patients with specific conditions under the supervision of a qualified practitioner.
- Monitor patients and promptly alert a qualified practitioner when there are unexpected changes.
- NVQ 3 or equivalent knowledge and experience demonstrated in application.
- Good general education including GCSE Grade C in English and Maths.
The estimated annual salary for this role is approximately £24,071 - £25,674.
