Social Worker

We are seeking an experienced Social Worker to join our multi-agency team supporting disabled children and their families. As a key member of our team, you will work alongside colleagues from health and education to deliver the best possible services for disabled children.
Your role will involve undertaking assessments underpinned by our trauma-informed strength and relationship-based approach, child-focused plans, and reviewing short break care packages. You will work with a diverse caseload, including child in need and child protection cases, as well as looked-after children, and coordinate their transition to adult services.
About YouWe are looking for a skilled Social Worker who can undertake effective assessments of need in an innovative way and coordinate the provision of appropriate services. You will have a strong understanding of the needs of disabled children and their families and be able to work collaboratively with colleagues to deliver high-quality services.
