About the Role:

We are seeking a highly skilled Children's Continuing Care Case Manager to join our CAMHS/CALDS team. As a key member of the team, you will play a vital role in ensuring the seamless provision of integrated services and packages of care for individuals who meet children's continuing care criteria.

Main Responsibilities:

  • Provide a case management role to a caseload of patient/service users.
  • Contribute to the development and implementation of an appropriate package of care.
  • Monitor clinical compliance, implement a robust review programme, and provide ongoing guidance and support to nominated Care Coordinators.
  • Support the decision-making process by chairing multi-disciplinary team meetings.
  • Ensure that commissioning decisions are lawful and reflect national priorities and agreed national and local service strategies.
  • Provide direct support to Care Coordinators in developing new referrals for Children's Continuing Care applications.
  • Quality assure Children's Continuing Care applications for patients/service users on colleagues' caseloads.
  • Attend the team Quality Assurance/Peer Review Panel to present cases and follow up on actions as necessary.
