  • Provide regular supported conversations with patients, assessing their holistic needs and providing relevant information and advice.">
  • Co-ordinate care and develop personalized care plans with patients, ensuring they have access to the right help at the right time.">
  • Assess and interpret patient needs, responding effectively to their requirements.">
  • Demonstrate ability to recognize and respond appropriately when faced with a sudden deterioration or emergency situation.">
  • Liaise with clinical staff to support patients and carers in distress, maintaining confidentiality at all times.">

  • We are seeking an individual with knowledge of health and social care environment, able to use Microsoft Office and databases.">
  • Able to work as a team, possessing excellent interpersonal and communication skills, and intermediate IT skills.">
  • Demonstrate ability to use own initiative, prioritize tasks, and adapt to changing situations.">
  • Experience of working within a team, with relevant health or social care experience at AFC band 3 or equivalent.">
  • Counselling qualification/experience desirable, along with willingness to undertake Macmillan training where appropriate.">

Salary and Benefits

">

The estimated salary for this position is £25,000 - £30,000 per annum, depending on experience. The role also offers access to a comprehensive benefits package, including pension scheme, life assurance, and annual leave entitlement.
