About the Role:

Duties and Responsibilities:

  • Carry out comprehensive assessments, applying the Care Programme approach (CPA), to identify suitable care levels and requirements.
  • Deliver person-centred assessments, incorporating evidence-based practices and considering individual needs.
  • Foster relationships with local communities, charities, and emergency services to enhance support networks.
  • Evaluate patient risk, inform care decisions, and highlight potential dangers.
  • Lead on social care initiatives, staying abreast of Safeguarding Children & Adults policies and best practices.
  • Maintain meticulous, up-to-date records, respecting client confidentiality.
  • Engage with ongoing research and advancements in mental health care.
  • Oversee client caseloads, providing Social Care interventions and collaborating with secondary mental health services.
  • Attend regular supervision sessions and participate in necessary training.
  • Prioritise patient protection, utilise public resources effectively, and adhere to relevant regulations.
  • Perform additional tasks as assigned by senior staff members.

Qualifications and Skills:

  • A recognised Social Work qualification or equivalent required.
