Experienced Healthcare Assistant for Primary Care Network

Job Summary: We are seeking an experienced healthcare assistant to join our primary care network team. The successful candidate will support our team members and work closely with mental health professionals to ensure individuals with severe and enduring mental health needs receive annual health checks and referrals to specialist services as needed.
Key Responsibilities:
- Support team members and work collaboratively to achieve team goals
- Conduct health screens for service users with severe mental illness and transfer records to GP databases
- Complete administrative tasks, including booking appointments and sending reminders
- Liaise with other healthcare professionals to ensure seamless care coordination
Requirements:
- Experience working in a healthcare setting, preferably in mental health
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Basic computer skills and familiarity with EMIS
