We are seeking a highly skilled and experienced Band 6 Specialist Practitioner to join our District Nursing team. As a key member of the team, you will be responsible for managing the routine activities of the District Nursing Team, organizing workload and staff rotas, and exercising leadership and judgement to ensure good clinical practices and high standards of care are maintained.

The successful candidate will have a strong background in community care and will be able to demonstrate leadership skills and attributes. They will also have knowledge of factors requiring consideration in planned change and be able to manage complex needs assessments and plan future care.

The role involves working within an integrated neighbourhood and being involved with current changes and developments within the organization. You will be responsible for assessing and developing students and junior staff to reach their potential by designing learning programmes and acting as a mentor.

Main Duties and Responsibilities:
  • Continuing daily responsibility and accountability for the District Nursing team without direct supervision.
  • Assessing, planning, implementing, and reviewing routine through to complex care packages using research and evidence-based knowledge to underpin practice.
  • Supervising, leading, informing, and delegating staff as appropriate to the needs of the service by utilizing Nursing skills and resources effectively as directed by the Team Leader.
  • Applying specialist, analytical, and judgemental skills to assess and interpret acute, chronic, and palliative conditions, taking appropriate actions to prioritize patients' needs.
  • Demonstrating a high level of intuitive and creative problem-solving skills to facilitate patient-centered care: rationalizing information to create patient concordance by using a high level of communication/negotiation skills.
  • Participating in the Performance and Development Review process within the team and undertaking review of junior staff under supervision of the team leader.
  • Prescribing efficiently and effectively from the Nursing Prescribing Formulary.
  • Participating in the investigation of complaints and safeguarding concerning nursing services in accordance with the level of experience and encouraging comments and suggestions on improving the quality of the service.
  • Ensuring that accidents, complaints, and unusual incidents are reported in accordance with the policy.
  • Analyzing caseloads and constantly reviewing work patterns as required.
  • Participating in systems of clinical governance to achieve quality improvements, i.e., clinical audit and evidence-based practice.
  • Supporting and developing staff using CPD and identifying future leaders.
Person Specification:
  • Registered General Nurse
  • Holds the Community Specialist Practitioner qualification (DN Pathway)
  • Registered non-medical prescriber
  • Registered mentor/Practice teacher
Experience:
  • Demonstrating leadership skills and attributes.
  • Knowledge of factors requiring consideration in planned change.
  • Ability to manage complex needs assessments and plan future care.
  • Demonstrating an understanding of clinical governance and safety/quality initiatives pertaining to the role.
  • Implementation of audit/evidence-based recommendations.
  • Excellent communication skills and awareness of conflict management.
  • Evidence of development needs/support/mentorship to junior staff including students.
  • Leadership module/study.
  • Evidence of innovative practice.
Skills & Knowledge:
  • Able to demonstrate a high standard of clinical practice and expertise to meet service need.
  • Knowledge of the values underpinning the NHS Constitution.
  • Demonstrating an understanding of national and local strategic direction within District Nursing.
Personal Qualities:
  • Ability to travel.
