Job Summary

We are seeking a skilled and dedicated Community Healthcare Assistant to join our team in the District Nursing OOH Service. As a key member of our multi-disciplinary team, you will play a vital role in delivering high-quality patient care and supporting our patients with Long Term Conditions and End of Life needs.

Key Responsibilities
  1. Collaborate with the Team: Function as an effective member of the multi-disciplinary team, working closely with colleagues to provide comprehensive patient care.
  2. Documentation and IT Systems: Maintain clear, concise, and legible documentation, adhering to prescribed standards and protocols, and utilize IT systems for relevant tasks, such as accessing patient records and results.
  3. Consent and Reporting: Obtain consent in accordance with Trust Consent Policy and report accidents/incidents on the clinical incident reporting system.
  4. Phlebotomy and Clinical Tasks: Undertake phlebotomy duties as delegated and overseen by the registered nurse, working within Trust policies and procedures, and perform clinical tasks as delegated by the Community Nurse team.
  5. Long Term Conditions and End of Life Care: Assist the Community Nursing Team in managing patients with Long Term Conditions and in End of Life care, recognizing the need for referral and liaising with other healthcare workers as necessary.
  6. Induction and Orientation: Participate in the induction and orientation programmes for all grades of staff who require community experience, as arranged by the Team Leader.
  7. Phlebotomy Stock Management: Maintain phlebotomy stocks, keeping them clean and in good order, and follow Trust policy for needle stick incidents.

Requirements: To be successful in this role, you will need to maintain an up-to-date knowledge of blood tests performed and select the appropriate sample tubes for analysis, ensuring they are correctly labelled at the time of sampling.
