Job Overview

Our organization is seeking a skilled Social Connection Specialist to join our team. In this role, you will work closely with patients, community groups, and local agencies to promote social prescribing and improve health outcomes.

As a Social Connection Specialist, you will be responsible for:

  • Supporting patients in developing a holistic approach to their health, connecting them with community groups and statutory services
  • Building relationships with key staff in GP practices within the local Primary Care Network (PCN)
  • Developing strong links with all local agencies to encourage referrals and ensure they have the confidence to make appropriate referrals
  • Working in partnership with local agencies to raise awareness of social prescribing and its benefits
  • Encouraging self-referrals and connecting with all local communities, particularly those that statutory agencies may find hard to reach
Responsibilities

As a Social Connection Specialist, your responsibilities will include:

  • Supporting existing groups to be accessible and sustainable, and helping people to start new community groups
  • Referring patients to non-medical link workers to support them in taking a holistic approach to their health
  • Working collaboratively with all local partners to ensure community groups are strong and sustainable
  • Encouraging people who have been connected to community support through social prescribing to volunteer and give their time freely to others
  • Providing regular confidence surveys to community groups receiving referrals to ensure they are strong, sustained, and have the support they need
Requirements

To be successful in this role, you will need:

  • Experience of working directly in a community development context, Adult Health and Social Care, Learning Support, or Public Health/Health Improvement
  • Experience of working with the Voluntary sector, including with volunteers and small community groups
  • Experience of partnership/collaborative working and building relationships across a variety of organizations
  • Able to communicate effectively, both verbally and in writing, with people, their families, carers, community groups, partner agencies, and stakeholders
