We are seeking a Carer Wellbeing Coordinator to join our team at Dccarers in County Durham. The estimated annual salary for this role is £16,022.

About the Position

This is an exciting opportunity for a dedicated individual to make a real difference in the lives of carers. As a Carer Wellbeing Coordinator, you will deliver information and support to carers through various channels, including telephone, online, and face-to-face appointments.

Key Responsibilities
  1. Delivering Information and Support: Deliver information, support, and advice to carers via telephone, in-person meetings, email, written documents, and virtual platforms such as Zoom, Teams, and Skype.
  2. Providing One-on-One Support: Provide individualised support to carers as needed and conduct support assessments for carers accessing DCCS services using the Carers outcomes tool.
  3. Caseload Management: Manage your own caseload, maintaining up-to-date records and monitoring as required using Charitylog.
  4. Referral and Signposting Services: Refer carers to Social Care Direct for comprehensive carer assessments when appropriate and direct carers to relevant organisations that can offer additional assistance based on their individual needs.
  5. Identifying and Engaging with Hidden Carers: Connect with GP surgeries, social care services, libraries, and community services within the allocated area to identify hidden carers and provide them with information about DCCS services.
  6. Supporting Carers with Welfare Benefits: Help carers access appropriate welfare benefits to maximise their income and support their caring roles, including assistance with form filling.
  7. Grant and Bursary Support: Support carers in financial hardship with grants and bursaries for essential household items.
  8. Staying Informed: Keep up to date with changes in welfare benefits, carer policies, and service provisions.
  9. Collaborative Working: Work collaboratively with fellow carer support coordinators to share knowledge and skills.
  10. Meeting and Training Requirements: Attend meetings, training sessions, and events as required.
